Pursuant to art. 13 clause 1 and clause 2 of the Regulation of the European Parliament and
of the Council (EU) 2016/679 of 27
th April 2016 on the protection of natural persons with regard
to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (GDPR), I hereby inform that:

  1. The Controller of your personal data is Dariusz Świątczak running business activities under
    the business name "FIRMA DALEKS Dariusz Świątczak", ul. Piotrkowska 62 lok. 7A, 90-105 Łódź, Tax Identification No. 982-000-75-37, Business Statistical No. /REGON/ 100222878, hereinafter referred to as the
    "Controller".

  1. Your personal data shall be processed only for the following purposes: execution of the lease contract and activities directly or indirectly related thereto, tax purposes; it may also be processed with regard to claims resulting from the civil law regulations, if any such claims occur.

  2. The legal basis for your data processing is art. 6 clause 1 letter b of GDPR (i.e. processing is necessary to execute the lease contract that you are a party of or to take certain actions upon your request prior to entering into the contract), art. 6 clause 1 letter c of GDPR (i.e. processing is necessary to fulfil the legal obligation I am subject to, e.g. notifying the Internal Revenue Office about the contract) and art. 6 clause 1 letter f of GDPR (i.e. processing is necessary to establish, pursue or secure claims).

  3. Your personal data shall be processed in paper documentation and electronic systems for the purposes related to the lease contract, as well as for the purposes of ensuring safety and proper operation of the IT system, e.g. in relation to making backup copies, identifying irregularities and ensuring protection against abuse.

  4. Providing your data is voluntary, however, it is necessary for the purpose of concluding and executing the lease contract. Failure to provide your personal data shall result in inability to enter into the contract.

  5. The recipients of your personal data shall only be entities authorized to obtain personal data under commonly applied law, including the Head of the Internal Revenue Office, the Lodz City Municipality (the City of Lodz Office), as well as entities providing accounting, IT, legal and cleaning services for me.

  6. Your personal data shall be processed throughout the term of the lease contract execution and thereafter, i.e. until the elapse of the limitation period for any claims resulting from the contract, as well as limitation period of tax obligations.

  7. The Controller shall not transfer your data to any third country or any international organizations.

  8. You have the right of access to your personal data or the right to demand its deletion, correction, rectification, restriction of its processing, the right to data portability, to file an objection to data processing, to withdraw your consent at any time.

  9. In relation to your personal data processing by the Controller, you have the right to file an objection with the supervisory body, i.e. the Head of the Personal Data Protection Office.

  10. Based on your personal data the Controller shall not take any automated decisions in relation to you, neither any decisions resulting from profiling.*

* Profiling shall mean any form of automated processing of personal data that involves using personal data for assessing some personal factors of a natural person, in particular for analyzing or predicting aspects regarding the work of the natural person, their financial situation, health, personal preferences, interests, credibility, behavior, location or mobility.
